For ALL Brave SC Participants

We use e-waivers only.

If you previously completed The Villages SC waiver for either yourself or a minor you are the guardian of and there are NO CHANGES you do not need to submit a new form. Once a minor turns 18 they will need to complete the Adult waiver even if a previous Minor waiver was submitted.

Adult Waiver – Ages 18 and older

To be signed by the adult participating in any Brave SC activity.

Minor Waiver – Ages 17 and younger

To be signed by the legal guardian/parent of a minor participating in any Brave SC activity.
